  • Overview


    The Registered Nurse is a qualified person who provides direct skilled care nursing services to patients in the home setting. The RN also coordinates care and services for a designated group of patients in the role of case manager. The RN supervises the Licensed Practical Nurse staff and Home Health Aide staff as required by regulation and policy.



    • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit
    • CPR certification
    • Mobile Driver - Valid driver’s license and automobile insurance per Company policy
    • Reliable transportation to meet visit schedule
    • Minimum one (1) year in-patient nursing, hospice, or home health experience for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate
    • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
    • Ability to use equipment with visual and auditory mechanisms
    • Ability to effectively communicate (written and oral) in English
    • Flexible schedule to meet the needs of patients
    • Ability to physically access any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) in any area Company serves

    • Clinical
    • Demonstrates excellent clinical assessment skills, adequate for accurate completion of the   Outcome and Assessment Information Set all versions.
    • Ability to perform necessary subsequent clinical assessment on a continuing basis to determine patient status and progress towards treatment goals.
    • Completes an initial assessment of patient and family to determine home care needs.
    • Provides a complete physical and psycho-social assessment and history of current and previous illness(es).
    • Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan, which establishes goals based on nursing diagnosis and best practices, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
    • Demonstrates excellent clinical procedure skill level.
    • Provides skilled nursing services to patients as indicated by physician orders, to include treatments and diagnostic procedures requiring specialized skill.
    • Initiates preventative and rehabilitative procedures as appropriate for patient safety.
    • Administers medications and treatments as prescribed by physician.
    • Observes clinical symptoms, reaction to treatment regimen and emotional status. Provides counseling to the patient and family relative to nursing needs.
    • Reports pertinent changes in patient condition to the physician. Obtains orders for deviations from the Plan of Care. Reduces verbal physician orders to writing in a timely manner. Reports pertinent changes to their supervisor in a timely manner.
    • Maintains accurate timely documentation of all clinical activities.
    • Provides supervision of assigned staff (LPNs/Home Health Aides), completes appropriate documentation to support supervisory requirements.
    • Is responsible for day-to-day coordination of clinical activities for an assigned group of patients, to include care planning, goal setting and physician liaison.
    • Maintains confidentiality of patient and company matters.
    • Accept responsibility and is accountable for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision when needed; perform within the guidelines established by the Florida Nurse Practice Act.


    Pain and Symptom Management

    • Assesses patient for pain at each visit.
    • Anticipates barriers to pain and symptom management and provides appropriate education
    • Bases interventions on patient/family goals
    • Re-assesses and adjusts intervention based on response
    • Identifies differences in goals of curative and palliative care
    • Patient’s wishes are respected and taken into consideration when planning and providing care


    Infection Control

    • Identifies signs and symptoms of infection, documents and notifies physician of any adverse findings.
    • Adheres to principles of infection control including standard precautions for all patients
    • Identifies conditions when transmission based precautions are needed
    • Maintains aseptic technique at all times.
    • Disposes of hazardous waste properly, including safe disposal of sharps.
    • Uses proper hand hygiene.



    • Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others.
    • Direct and supervise patient care provided by other nursing team members.
    • Participate in orienting new staff.
    • Participate on committees and projects, as assigned.
    • Act as a resource to peers, orientees and other staff members.
    • Participate in activities and professional development that enhance home health nursing skills.Professional Development 
    • Pursue own professional growth through attendance at required in-services (i.e., safety, infection control, Right to Know Law) and other educational offerings.
    • Establish professional goals for self and continually evaluate self in the attainment of these goals..

    • Participate in interviewing and selection process of new hires.
    • Provide input to Clinical Team Supervisor for LPN and Home Health Aide job performance.
    • Perform other duties as assigned.
