    Role:

    The Palliative Care Registered Nurse acts as a coordinator and facilitator for Palliative Care patients.

     

    Qualifications:

    • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit
    • Mobile Driver - Valid driver’s license and automobile insurance per Company policy
    • Reliable transportation to meet visit schedule
    • Minimum of one (1) year RN experience (in-patient, hospice, or home health)
    • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
    • Ability to use equipment with visual and auditory mechanisms
    • Ability to effectively communicate (written and verbal) in English
    • Flexible schedule to meet the needs of palliative care patients
    • Ability to physically access any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) in any area Company serves
    • Ability to manage the emotional stress of working with and caring for palliative care appropriate patients and their families
    • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodationCompetencies:

    • Participate in Quality Assessment and Performance Improvement activities as appropriate for the position.Job Responsibilities: 

     

    • Collaborates with referring providers, palliative care providers, and the HIM Technician to identify and facilitate the scheduling of newly referred patients.
    • Obtains initial new patient intake assessment data, primary diagnosis, chief complaint, past medical history (PMH), family history, social history, physical symptoms, medications and education needs.
    • Assists in scheduling initial and follow-up appointments.
    • Provides patient education including: contact information, after hours’ coverage, medication questions, prescription renewal process, and follow-up appointments.
    • Provides on-going nursing assessment of the patient across the continuum in relations to treatment plans, patient and family inquires, symptom management, psycho-social support, and education. May initiate a Plan of Care based on protocols and algorithms.
    • Completes, processes and follows-up on prescription and prior authorizations.
    • Assists medical team with telephone calls and triage to provide safe, effective and appropriate disposition for health-related problems, using approved practice protocols. Assists medical team with referrals to home health and hospice.
    • Provides nursing interventions, where appropriate. Obtains vital signs and weight (if appropriate) when patients arrive for their clinic visits and, if requested or required, change wound dressing per order of the medical staff.
    • Provides support to Palliative Care SNF team. Duties to include gathering pre-visit data, identifying and locating family members for meetings, and other undertakings that will assist the APRNs.
    • Performs other duties as assigned.
